- I have completed the Preliminary Rescue Adoption Application. If there are any changes in my information on that application, such as marital status, address, telephone, email address, etc., I will note them at the bottom of this Agreement.
- I hereby agree that the Cavalier King Charles Spaniel being placed in my care will exclusively be kept as a house pet as part of the family and will never be kept out of doors or left outside unattended for any extended period of time. If not in a securely fenced area, it is vital to keep your Cavalier on a leash at all times when outdoors. Cavaliers will run after birds, squirrels, rabbits, and because of their friendly nature they do not understand the danger of running up to a strange dog (no matter how large).
- Health, behavioral and background information to the extent known, is attached to this Agreement and incorporated herein by reference.
- I understand that this dog has been spayed or neutered or because of age or health condition, is placed on a temporary medical exemption upon the recommendation of a veterinarian: I agree to follow Dr. Jean Dodds Vaccination Protocol which is attached to this Agreement and incorporated herein by reference. This protocol does NOT include vaccinating on an annual basis. I understand that vaccinations are advised only for healthy dogs and if my dog should develop a heart murmur, they are no longer considered healthy. Instead of vaccinating annually, I understand it is recommended I do titer testing and will contact the Rescue Trust if I have any questions about this. I agree to give other medications as directed by my veterinarian.
- In the event circumstances develop which would prevent me from keeping this dog, I will immediately contact the ACKCS Rescue Trust for assistance. I will NOT give, sell or transfer this dog to any person, animal shelter, pound, humane organization, rescue or veterinarian .
- ACKCS Rescue Trust assumes no liability for veterinary expenses incurred or liability for claims or damages during the tenure of this placement and I hereby release the American Cavalier King Charles Spaniel Rescue Trust, it's agents, volunteers, their heirs, and all others previously involved with this dog, whether known or unknown, from any liability incurred from this day forward. Further, I will assume all veterinary expenses from this day forward.
- I understand that the ACKCS Rescue Trust reserves the right to repossess this dog (at my expense) should any portion of this, or any other contract or agreement between me and the ACKCS Rescue Trust be breached by me, or if I have been found to have neglected or mistreated this dog. Furthermore, if the dog is ever lost or abandoned or turns up in a shelter then this contract is void and ownership of this dog reverts back to the ACKCS Rescue Trust. I certify that all of my answers on this and any other contract or agreement between the ACKCS Rescue Trust, and me are true and correct and that falsification will breach these contracts or agreements.
